How It Works

Step 1:Request Your Appointment

Step 2:Complete Your Secure Intake Forms

After you submit your request, you will be guided to complete your secure intake and consent forms.

Please complete all forms as soon as possible so we can schedule you quickly.

Step 3:Set Up Your Patient Portal (OnPatient)

You may receive an email invitation to create your OnPatient (DrChrono) portal account.

This portal allows you to:

  • Sign forms
  • Review instructions
  • Manage your telehealth care

(Tip: If you don’t see the portal email, check your Spam/Junk folder.)

Step 4:We Schedule Your Visit

Once your forms are complete, we will schedule your visit — often within 24 hours when possible, or at your preferred available time.

Step 5:Prepare for Your Telehealth Appointment

Before your visit, please have:

  • A smartphone, tablet, or computer with camera and microphone
  • Reliable internet connection
  • A private, quiet space
  • Photo ID and insurance card (if applicable)
  • A list of your current medications
